Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (PM-JAY) is the world largest health insurance scheme. It provides a cover of Rs 5 lakh per family per year for secondary and tertiary care hospitalization.

- Families listed in SECC 2011 data
- Registered on AB-PMJAY portal
- Rural and urban poor families
How to Apply
- Visit beneficiary.nha.gov.in
- Check your eligibility using mobile number or ration card
- If eligible, apply at nearest CSC center or Ayushman Mitra
- Get your Ayushman Bharat card
